Aortic Perigraft Seroma Following Total Arch Replacement

چکیده
Perigraft seromas (PGS) are a rare but important complication following aortic surgery. This study presents experience of proximal PGS in young male fourth redo cardiac surgery, and reviewed the published literature on management PGS. A case report review all cases 25-year-old with Marfan syndrome underwent surgery for completion total arch replacement via clamshell thoracotomy. mediastinal fluid collection around distal aspect graft was first noted 6 months postoperatively computed tomogram angiogram. Extensive investigations including diagnostic aspirate were performed to determine aetiology collection. Although initially asymptomatic, patient became symptomatic years postoperatively, measuring 115mm x 93mm 110 mm. He an exploratory left anterior mediastinotomy, where diagnosis confirmed. Reports complicating grafts exceedingly rare, four other described literature. It is believed that this largest reported PGS, only intervention mediastinotomy. Proximal perigraft collections can represent challenge. Stable asymptomatic patients normal inflammatory markers may be suitable conservative close surveillance. guideline investigation collections.

Total endograft replacement of aortic arch.
Total endovascular replacement of the aortic arch is a complex procedure that is often favoured when the pathology anatomy precludes a standard median sternotomy. Here we present the case of endograft repair in a 79 year old male with 6.5 cm arch aneurysm and 5.4 cm descending thoracoabdominal aneurysm. Following bilateral carotid-subclavian bypasses, a long 7 Fr sheath was advanced into the de...
متن کاملTotal arch replacement for distal enlargement after ascending aortic replacement for acute type A aortic dissection.
OBJECTIVE Distal reoperations for aortic dissection are associated with high morbidity rates. We describe distal aortic enlargement that was treated using our surgical strategy. PATIENTS AND METHODS From January 1997 to April 2008, 63 patients underwent ascending aortic replacement for acute type A aortic dissection. Four patients (7.4%; 3 males, 1 female; mean age, 67.8 +/- 4.6 years) requir...
متن کاملTotal arch replacement using antegrade cerebral perfusion for distal aortic arch aneurysm.
Total aortic arch replacement remains a complex surgical procedure, best practiced in high-volume aortic centers. The following report details the surgical procedure for total arch replacement with selective antegrade cerebral perfusion (SACP) for distal arch aneurysm. Following moderate hypothermic circulatory arrest and SACP, the distal arch, and then the proximal arch, is reconstructed, foll...
متن کاملSimultaneous cusp-sparing aortic root replacement and coarctectomy with total arch replacement from the midline incision.
Four cases of simultaneous surgery for aortic root aneurysm with aortic regurgitation and coarctation of the aorta were presented. Age at surgery ranged from 18 to 37 years and all were male. All had annuloaortic ectasia and dilatation of the ascending aorta, 3 had bicuspid aortic valve and 1 had acute localized aortic dissection. Preoperative grade of aortic regurgitation was trivial in 1, mod...
